Printing Instructions

Use only the PDF survey file as a master copy from which to make copies. (Using copies to produce more copies will result in surveys that cannot be read by the scanner.)
The student and parent surves are each 4 pages in length; the educator is 3. Please produce a two-page front and back survey.

Some things to keep in mind when administering the surveys:

Timing on the student survey, keep in mind that administering the surveys in the spring will more accurately reflect the impact of this year's GEAR UP activities on your students.
Using the same time frame each time you administer the survey will give you more comparable data.
Surveys are due to NWREL no later than March 1.
